What we do

The main purpose of the Forum is to create an interagency partnership that will promote effective transitions into adulthood, for young people with special needs and disabilities.

We have a Terms of Reference.

We wrote a Transition Protocol to explain how transition planning should be for young people with disabilities or special educational needs in Brighton & Hove.

We are looking at how Transition could be done better in health services. We had a project worker do a Health Transition Report for us (here is a summary of the Health Transition report).
